Provides support and assistance with daily tasks and personal care for individuals in their residences as outlined in a personalized care plan.

• Adheres to the designated care plan and promptly informs the supervising nurse of any changes or unusual circumstances.
• Aids individuals by assisting with grooming, bathing, and dressing.
• Offers assistance with household chores, laundry, grocery shopping, snack preparation, and meal assistance as specified in the care plan.
• Aids in the mobility and transfers of clients, including helping them in and out of bed or chairs and supporting them with walking.
• Promptly seeks emergency help if necessary.
• Participates in all mandatory training sessions and in-service meetings.
• Maintains a professional demeanor and appearance at all times, wearing the company-provided identification badge and approved uniform during work hours.
• Provides care for pets by walking dogs, attending to bathroom needs, cleaning up waste, feeding pets, and other related tasks.
• Offers companionship by assisting with mail, reading aloud, engaging in conversations, and accompanying on walks.
• Acts as a companion during dining times and assists with escorting individuals to various activities within the premises.
• Proficiently uses required applications like Paylocity, Authenticare, MatrixCare, HHAExchange, and Carebridge.
• Other responsibilities as assigned.

• High School Diploma or equivalent is the minimum educational requirement.

• Current negative TB skin test or state-issued health card.
• Completion of a 75-hour Home Health Aide training program.
• Certification in CPR/First Aid.
