About HealthPlus!
Summary :
The Wellness Program Coordinator conducts wellness program sessions. Assists participants in identifying health risks, interpreting data, and creating individual wellness plans. Ensures the delivery of patient care that aligns with HealthPlus’s high standards through teamwork. Contributes to and is responsible for offering compassionate, secure care and exhibiting dedication to continuous enhancement and professional advancement.
Job Description:
KEY RESPONSIBILITIES:

Evaluates health assessment data to help clients comprehend health risks, readiness for change, goal setting, and effective strategies for achieving those objectives.

Offers guidance on enhancing lifestyle behaviors through understanding the impact of lifestyle choices on health risks.

Maintains an accurate participant list for billing purposes.

Collaborates with internal and external partners to advocate wellness initiatives and foster a supportive wellness atmosphere.

Interprets and communicates assessment data for clients.

Leads on-site wellness sessions to ensure efficient patient flow and punctuality, providing quality service.

Adheres to HealthPlus and external safety regulations to maintain a safe work environment while at non-HealthPlus sites.
REQUIREMENTS:

High School diploma or GED is preferred

Minimum age requirement: 18 years

American Heart Association Basic Life Support (BLS) or CPR certification for Healthcare
PHYSICAL REQUIREMENTS:
-
Must be able to move freely (standing, stooping, walking, bending, pushing, pulling) and lift up to twenty-five (25) pounds without assistance

Job may involve exposure to bloodborne pathogens (blood, bodily fluids) during tasks

Manual dexterity and hand-eye coordination for patient care procedures
WORKING ENVIRONMENT:
-
Office setting with controlled climate, interspersed with movement around the facility. At non-HealthPlus sites (on company premises), may be required to wear safety gear like hairnets, ear protection, steel-toed shoe covers, etc.

Interaction with various department members and healthcare professionals

Periodic exposure to mechanical equipment, fumes, airborne particles, and hazardous substances

Occasional high noise levels in the workplace

Possible risk of exposure to infectious diseases, dangerous substances, and medications
