Job Title: Remote Patient Care Coordinator, Health Services Department: Regional Healthcare Support Center Objective: Responsible for coordinating patient care remotely and ensuring adherence to treatment plans. Primary duty is to provide guidance and support to patients in following prescribed medical protocols. Will utilize telehealth platforms for virtual consultations. Reporting To: Supervisor, Regional Healthcare Support Center Management Responsibility: None Supervision: This role does not involve supervisory duties or the need to direct others. It is primarily focused on independent tasks. Resource Management: Minimal. The position involves minimal responsibility for managing resources, such as personal work tools or small supplies. While occasional errors may occur, they typically have minor cost implications for correction. The scope and types of resources involved are limited. Challenges related to resource management are straightforward. Key Connections: Team Members/Healthcare Staff, Clients, Community Members, Patients, Families, Partners POSITION REQUIREMENTS Education: Bachelor’s degree preferred Major Field Of Study: Healthcare Administration or related field Additional Training: Preferred training in telemedicine Licensure/Registration: Not required Experience: At least 1 year of relevant experience preferred People Skills: Interactions primarily occur with colleagues in the immediate work environment. Communication is generally routine and uncomplicated. The job demands basic interpersonal skills and courtesy for effective communication. Work Environment: Occasional exposure to challenging environmental conditions, safety hazards, and uncomfortable surroundings. Adherence to safety protocols is necessary.
